Once you decide that you would like to work with SDM, please use the calendar to schedule appointments. Your privacy is very important to us. Assessments
In-Take Session
Consultation
After the free consultation session, you will receive an estimate in writing providing the final evaluation fee.
For private pay evaluations, at the first in person test Session appointment, a 50% payment is required and processed. The remaining balance is paid when the results appointment is scheduled. A report will not be written until full payment has been received.

Consultation and Intake Session Fee is $125 per hour, and may be built into the final cost should you move ahead with services.
Services
For our Services with our network contracted providers, the fees are typically set at per hour cost or grouped as session bundles. Bundles provide a 10% off the regular session price. Payment is expected at time the appointment is made.

We want to make the payment process as easy as possible. We accept VISA, Mastercard, and cash. You can pay securely with your Credit Card.

Supporting Diverse Minds is an out-of-network provider. You are encouraged to call your insurance company to see if they accept a Super Bill for assessment services completed. You may be able to receive 50% of the cost returned to you by the insurance company. We may be able to help you with codes requested by insurance companies. ​​
